WE DID IT!!

This month seven communities will complete the LiveWell virtual learning collaborative. Despite the challenges affecting almost every long-term care community — low staffing levels, staff absence, and low census — these communities found a way to make significant progress, build their teams, and boost morale. Here are a few of their accomplishments:

Emerson House Memory Care

  • No falls. They stopped tracking falls after a month because they weren’t having any!

  • More showers. Shower compliance increased from 9% to 50% in just one month!

  • Almost no medication errors. After more than a month with no medication errors, they stopped tracking. When one happened, they did a root cause analysis using the 5 Whys tool.

  • Improved on-time attendance for the night shift

Nehalem Valley Care Center

  • More staff participation. Staff voted on measures to track.

  • Stronger community. Staff and residents are complimenting one another and learning about each other using the Who am I tool.

  • Better communication. Staff are starting to use the SBAR tool to quickly and effectively communicate resident status.

Pacifica Senior Living in McMinnville

  • No more absences! They went three weeks without a callout.

  • Falls are declining. Falls decreased 40% in April from their Jan., Feb., and March totals. They’re producing trend charts for falls and med errors to show staff the positive resident outcomes.

  • Staff appreciation. Staff and residents are using compliment cards all the time.

Rose Linn Vintage Place

  • Reduced medication errors

  • Re-worked the medication carts so that they have the same supplies in the same spot

  • Tracked staff training

Fellow cohort members Maryville Memory Care, Macdonald Residence, and Rock of Ages are building teamwork, communication, and understanding of the LiveWell Method in their communities as well.

We're so impressed with their continuous improvement and wish them even more success in the months to come!

Virtual Learning Collaboratives

Our virtual learning collaboratives provide training and coaching in a peer-learning environment. Participants learn three foundational practices of quality improvement.

Meet Ups = Community + Content

The LiveWell team is committed to building community among staff working in long-term care communities. One way we do this is by holding 30-minute Meet Ups on the 2nd and 4th Tuesday of every month from 10 am-10:30 am. They are open to anyone. They offer a forum for staff to talk about current topics such as COVID, staff retention, end of life planning, diabetes management, and dementia care.
